financial difficulties during and even after treatment.
We appreciate your willingness to help these cancer families
during the holidays.
Childhood Cancer Families: If you are a family that is just diagnosed, in treatment, finished treatment or has lost a child to cancer please download the application. Fill it out and fax it to (425) 820-9611.

Fill out the adopters document and fax it to (425) 820-9611. We will match you up with a family.
Office giving trees
If you have a giving tree in your office we will provide you with 3 X 3 wish tags.
Families at Mary Bridge Hospital
If you are currently being treated at Mary Bridge Children’s Hospital, please call your social worker or see them at your appointment for your application.
Families at Children’s Hospital
If you are a family at Children’s Hospital please down load the form and have you social worker sign it at your next appointment fax it over to the (425) 820-9611 fax number.
Families at Madigan Hospital
Please down load the form, fill it out and have your social worker sign it at your next appointment. Then fax it to (425) 820-9611
Remember when filling out the application please give as much detail as possible.
For example:
- Shoes (what kind) dress, tennis, soccer, football, baseball, hockey
- Shirts (what kind) dress shirt , play shirt, school shirts, color, size
- Pants (what kind) dress pants, play pants, color, size
Do you need anything that is so old, you need a new one? For Example: pots, pans, sheets, dish towels, bath towels, bath mats.
This is the time to add to your wish list something you have wanted to replace all year, and just have not gotten around to doing it.

Dear Childhood Cancer Careline,
Thank you so much for the gifts this year. Not only did you help us out with the things we were missing around the house; you also gave us gift cards to choose things for ourselves. The kids will get more than enough use out of them (especially the Barnes and Nobles ones), and I used mine to get things for our stockings and other things we have not had in a very long time (because I had no money).

"Just wanted to thank you for all your help this year. We received the gifts... it was awesome for all the kids to be able to have a Holiday. We couldn't believe all the stuff. What a blessing. The highlight was when you had arranged for our local Windermere office to stop by with a Santa, the kids were so excited. Have a great Christmas.
